Yolo Adult Day Health Center offers services to caregivers via the Caregiver Support Program (CSP). The program provides care coordination assistance by working with the family caregiver to identify and access needed services. In addition the CSP provides counseling, community education and support groups to the community at large. The program seeks to address the stress often associated with the responsibilities of caregiving for a loved one and to maximize the time the individual can remain in their home.

The Elder Abuse Prevention Project provides training for paid workers in Residential Care Facilities for the Elderly (RCFE’s) including instructions on how to complete mandated elder abuse reports of suspected abuse.
The Ombudsman Program provides information and complaint resolution for residents of long term care facilities in the seven counties of Nevada, Placer, Sacramento, Sierra, Yolo, Yuba and Sutter. In addition to paid staff, the ombudsman program recruits and trains an average of 65 volunteers who complete extensive training and volunteer at least 20 hours per month assisting residents in long term care facilities. Ombudsmen are trained to receive and investigate reports of suspected abuse.

The statewide Health Insurance Counseling and Advocacy Program (HICAP) assists individuals and families with Medicare problems and other health insurance concerns. Through individual appointments and through community education, volunteer counselors who are trained and registered provide objective information on Medicare, Medicare supplemental insurance, managed care, long-term care planning and health insurance. The HSNC office provides HICAP services in El Dorado, Nevada, Placer, Sacramento, San Joaquin, Sierra, Sutter, Yolo and Yuba Counties.

Our services provide a hot nutritionally balanced noontime meal to senior citizens, age 60 and older each weekday, including legal holidays throughout Yolo County. We function through two programs-the Congregate Program where seniors come to a senior/community center to eat and socialize and the Home Delivered Meals Program (Meals on Wheels) where meals are delivered to the doorstep of shut-ins. We serve meals in the communities of West Sacramento, Woodland, Davis, Winters, Knights Landing, and Esparto.

Funding supports transportation services for Davis and rural Yolo County residents to access and benefit from Yolo Adult Day Health Center. Yolo Adult Day Health Center serves adults 18 and older with medical or cognitive challenges that interfere with independent living. Over 75% of program participants are low income. All clients have chronic health care needs and are unable to perform at least two activities of daily living such as bathing, dressing, walking, eating and dressing. The program offers nursing, physical, occupational and speech therapies, social services, dietary services, activities, transportation and specialized support for family caregivers. This community-based multidisciplinary approach to care reduces ER visits, minimizes hospitalizations and prevents need for skilled nursing placement.
